All insurance forms are available in each Clerk of Court's office or
online. Please complete these
forms as soon as any changes occur in your insurance status. These changes may include but are not limited to:
- Marriage, birth of a new child, divorce.
- Change of address
- Job termination
If an employee who is a member of the Insurance Trust terminates a position with a Clerks of Court office, the Insurance Trust Department will forward information to the employee regarding continuation of coverage through the Consolidated Omnibus Budget Reconciliation Act (COBRA). This federal law requires employers to provide continuation of health care benefits to certain persons who would otherwise lose their coverage due to a "qualifying event." Life, Accidental Death and Dismemberment, and Disability Income coverage are NOT subject to COBRA.
The Insurance Trust department must receive a notification of termination on or prior to the date the employee terminates, so the information can be forward to the terminated member in a timely fashion. The terminated member then has ninety (90) days to elect COBRA benefits. This coverage is provided at the member's expense.
There are insurance booklets which outline the coverage benefits and options for our insurance programs. These are normally sent to each new member along with an identification card. The member can view the
Employee Enrollment Guide booklet online. If the member loses their identification card, they can call the LCCIT Member Services office at (800) 763-4868 to request a replacement. To replace a dental or life booklet, please call the LCCIT Member Services at (800) 763-4868.
Insurance programs are run by human beings and from time to time there may be problems, which leads to slow responses in claims or denied claims. Please call LCCIT Member Services to discuss any problems prior to calling the Insurance Department of the Clerks of Court Association.
